Online Forms for Circulation
Services at BGSU Firelands, and Main Campus Libraries
of these forms are submittable online via email. Other forms can be
edited online, but must be printed out and submitted in person, by mail
or by fax to the circulation desk of Jerome or Firelands library where
you intend to pick up the materials.

/ Turning in the forms and having them processed
forms require Adobe Acrobat
Reader 4.0 to be loaded on the computer you are using to view and
edit the forms. Using any other version of Reader may cause problems
with printing and editing. Adobe Acrobat Reader is free and you can
download it from Adobe over the Internet by clicking the icon below.
- To fill in the
forms online, put your cursor in the spaces provided for filling in
information. Use the "tab" key to move from entry to entry
or click in the next space. "Enter" or "Return"
will clear the field, not commit data! Some spaces meant for staff use
are not active and will not accept data. If you need to make a correction,
click back into the field to make it active and retype your corrections.
- Due to Acrobat
design features and library confidentiality policies, these forms clear
all information upon opening the form. Therefore, you cannot use the
back and forward buttons on your browser to copy and paste information
from the library catalogs or databases. You must open a second browser
window and keep two windows open, moving back and forth between them,
to copy and paste.
- Print out the
form using the custom buttons on the form or the Adobe Acrobat print
button (not your browser's print button.) Sign it if necessary, and
turn in, mail or fax to the circulation desk where you will pick up
the materials or where you do most of your library business.
- If the printer
you are using is set to "print as image" in the print dialog
box for some printers, you will experience extremely slow speeds at
- Some printer drivers
strip the spaces from preexisting text on the forms. As long as staff
can read the information you have entered, ignore what happens to the
rest of the text.
- Do not attempt
to e-mail these forms even if you think it is theoretically possible.
Opening the form clears the data, and staff will receive a blank form.

be submitted when you click on the send button..
To turn in the
form(s) go to either the circulation desk of Jerome Library or Firelands
Library, wherever where you will pick up the materials.
You must have
a current, valid record in the BGSU library patron database to have
most of these forms processed. Use "View
Your Circulation Record" to check if your record is valid. If
you are having trouble verifying the status of your record, call 419-372-2051.

- Proxy Borrower
forms must be signed by the faculty member and turned in before research
assistants or teaching assistants can pick up books in the faculty member's
name. Proxy borrowing is available for faculty only. Only teaching assistants
or research assistants may be named as proxy borrowers, and for no greater
a period than one academic year..
With the exception
of photocopied articles from Interlibrary Loan, Northwest Depository,
and Firelands, all requested material will be held at the circulation
desk where you turned in the form. Photocopies will be sent to the
address you provide on the form unless you request them to be held
for pickup also.
If you do not
have a record in the library patron database, if your record has not
been updated for the current semester, or the citation information
is incorrect, the request will be returned to you without processing.
