Here are some of most frequently used forms for journalism majors and minors. You can download them in Adobe Acrobat format and then print them. These forms are also available in the School of Media and Communication office, 302 West Hall.

Internship Policy (revised December 2012): Procedures and requirements for receiving journalism internship credit (JOUR 4000). Your adviser may have other requirements as well. Note that all paperwork for the internship must be completed and submitted BEFORE you begin working. Work done before the completion and submission of the internship application (see below) may not be counted for an internship. Note that all paperwork for the internship must be completed and submitted BEFORE you begin working. Declaring a Sequence: Use this form to declare a sequence and request major status after completing JOUR 2000. Please see your advisor for additional information.
Independent Study: Complete this form in duplicate and return to the School of Media and Communication office to enroll in an independent study course. Requires signature of supervising instructor.
Minor Form: Complete this form if you are declaring journalism as a minor. Requires advisor's signature.
