Temporary Telephone Service Activation Request Form


(Faculty and Staff only)

To submit a request for an installation of TEMPORARY telephone service, please provide the information requested below. Your department's budget administrator's approval is required before filling out this form. Based on information provided in this form, a case ticket will be generated, and you will receive an email confirmation. Your request will be completed as soon as possible. For information on the charges associated with this request, please see the telephone service charges page.

All temporary phone service will be restricted for long distance. If you need to make long distance calls, you will need to contact the Technology Support Center  (419-372-0999) to obtain a University calling card. This is not necessary to make local calls or calls to toll free numbers.
